The diagnosis of a chronic illness represents a major transition in an individual’s life. Life as it was is no more and will not be again. Imagine a patient with several chronic illness diagnoses and multiple prescribers. This patient has diagnosis of congestive heart failure, hypertension, and diabetes. There are many self-management care and health promoting lifestyle changes that accompany the diagnosis of these chronic illnesses that the individual will need to incorporate into his or her daily life.
