Unit Objectives:

  1. Develop effective interventions that enhance team spirit and to prevent incivility and horizontal violence.
  2. Examine the nurse manager’s role in applying conflict management strategies.
  3. Investigate how nurse managers and leaders prevent and/or mitigate moral distress.

Purpose: The purpose of this assignment is to apply conflict resolution skills using effective communication in the role of the nurse leader.

Situation:

You are the acting charge nurse for an urgent care clinic on the overnight shift (7P-7A). On shift with you are three registered nurses (RNs), one certified nursing assistant (CNA), and one emergency care physician (MD). The nurse manager has left the facility for the day. You have had an influx of patients this evening. By 11 p.m., all 20 beds are filled and you have 13 patients awaiting triage. Of the 20 patients, 3 of them need to be transferred to the local emergency room for more extensive care. You have one ambulance immediately available; two others will arrive in 15-30 minutes depending on traffic conditions.

As you plan to transfer patients to the local emergency room for more extensive care, you collaborate with your team members to determine the priority of transfer. Nurse Maggie believes her patient, who is showing signs of diabetic ketoacidosis should be transferred first. Nurse George believes his patient, who is showing signs of a COPD exacerbation should be transferred first. Nurse Maggie and Nurse George get into an intense argument over who is correct. Nurse Maggie gathers the MD and CNA at the nurse’s station to discuss the issue and how “wrong and ridiculous Nurse George is.” The argument escalates quickly between the two nurses. An enraged Nurse Maggie slams the chart against the table and says that if you do not side with her she is leaving the urgent care clinic immediately.

Assignment:

Part 1: Script Writing

Develop a 5-10 line script using cooperative strategies to resolve the conflict between you, Nurse Maggie, and Nurse George. Download the script template for this assignment.

Part 2: Essay Writing

Develop a 2-3 page explanatory essay that addresses the following information:

  1. Elaborate on the nurse manager’s role in conflict resolution and team building.
  2. Develop at least two interventions for decreasing incivility in the workplace that are supported by scholarly evidence.
  3. Discuss in detail at least two methods for decreasing moral distress in the workplace that are supported by scholarly evidence.
  • You must include at least three evidence-based, peer-reviewed resources to support your ideas. These include .gov and .org professional websites, journal articles, and your textbook(s).
  • You must include headings in your paper indicating each section. This is a key concept to professional writing. It is also a fantastic method for ensuring you have met all Assignment requirements. Purpose

Clinical practice is rife with ethical dilemmas, including those generated from patients, colleagues, and systems. The successful nurse leader must learn how to navigate through these difficult situations and minimize moral distress.

Assignment Parameters

Answer the questions and statements related to the case study. You will be graded on writing content, organization, style and mechanics. This Assignment must include a minimum of three (3) references to support your responses. Use the online resources to access your state’s Nurse Practice Act and the American Nurses Association website. Additional references can be from your textbook or the online library. Unit 8 Worksheet

The nursing supervisor on the night shift at the local hospital receives a call from the charge nurse on one of the medical-surgical units. The charge nurse is calling to report a situation where a registered nurse and a nursing unit assistant have exchanged photos of patients in embarrassing moments. The photos were taken using personal cell phones and without the patients’ knowledge or consent. Per hospital policy, the supervisor has confiscated the cell phones from the RN and the NUA and turned them over to the hospital administration.

As a nurse leader, you are sometimes confronted with ethical and legal dilemmas. Fortunately, multiple laws and professional standards are available to guide you in the best direction for the patient, the organization and yourself. (Your state’s Nurse Practice Act, the ANA Code of Ethics are great places to start)

For this discussion board assignment, please select one of the two scenarios below and answer the discussion questions related to the cases.

Scenario 1: Catching up on work

You are the new nurse manager of a busy medical office and you are asked to come to the corporate office because of an incident that occurred after hours yesterday.

A local citizen brought a shopping bag of patient charts to the area hospital. The charts were found at a car wash. They appear to be the charts of patients seen over the past few days in your office. An investigation revealed that a nurse who works in the office has been bringing charts home to complete documentation in the evenings and then bringing them back the next day. On this occasion, the nurse washed their car and removed the bag of charts to vacuum. The charts were then inadvertently left at the car wash, where the local citizen discovered them.

The nurse admitted that she took the charts home even though it is clearly prohibited by policy. She stated that she has been struggling to keep up with her workload because of frequent staffing issues in the office.

Scenario 2: False intentions

You are the nurse manager of a small pediatrics unit. When you arrive to work one morning, night Nurse A asks to speak with you. The nurse reports that a patient appears to have a serious infection in a wound on her face. The wound dressing on her face was identified by date and initials as the same one that Nurse A applied the night before. Nurse A reviewed the charting from the off-going Nurse B. Nurse B’s documentation indicated that the dressing was changed and the shift assessment revealed no signs of infection.

Upon the attending physician’s examination, the wound had spread and was presumptive for osteomyelitis. The patient was transported to a larger medical center for treatment. An interview with Nurse B revealed that the nurse had been “busy” and forgot dressing supplies when the shift assessment and medications were administered. Nurse B stated that the patient’s family was difficult to deal with and often scrutinized the nurse’s care. While charting at the end of the shift, Nurse B falsified documentation rather than face the family and have them question the nursing care.

Discussion Questions

  1. Please explain what violations of ethical standards or laws have occurred in the selected case. Include the specific language from the legal or professional entity.
  2. Determine whether the occurrence needs to be reported to a legal entity such as your state’s board of nursing or any federal agency. Include supporting rationale and the reporting process if applicable.
  3. Are there any legal or financial ramifications of the event in terms of fines, lawsuits or additional agency oversight? If so, what are they?
  4. What is your evaluation of the scenario? What could you do as a nurse leader to prevent a similar event from happening in the future?
